Complete all registration and medical history and surgery packet forms from the comfort of your home by using your Smartphone or PC. It’s easy just follow and answer the required questions. You will receive a text message/email (check spam folder) from us 2 days prior to your visit, from the same method used for reminder appointments. Complete all forms online and submit, easy as that. This is HIPAA compliant and safe and will assist in decreasing your wait time and have a safer check-in and updated medical history.

We accept checks/debit cards, Visa, MasterCard, American Express and Care Credit for your convenience.
We do not participate in vision plans.
If you participate with an insurance plan that requires a referral, please have the referral forwarded prior to your appointment. Your insurance company will not be responsible for your bill without a referral.
