We do not participate in Vision Plans.
If you participate with an insurance plan that requires a referral, please have the referral forwarded prior to your appointment. Your insurance company will not be responsible for your bill without a referral.

Please note we are now accepting digital intake forms for our patient’s safety and convenience. Complete all registration and medical history and surgery packet forms from the comfort of your home by using your Smartphone or PC. It’s easy just follow and answer the required questions. You will receive a text message/email (check spam folder) from us 2 days prior to your visit, from the same method used for reminder appointments. Complete all forms online and submit, easy as that. This is HIPAA compliant and safe and will assist in decreasing your wait time and have a safer check-in and updated medical history.
In order to expedite our check-in procedures, please review and complete these forms prior to your visit and email to location you will be visiting Frontdeskdallas @eyesurgeonsdallas.com or frontdeskplano@eyesurgeonsdallas.com.
If you have filled these out in the past, we request that you update these forms so our records are current and correct for each visit.
