We do not participate in Vision Plans.
If you participate with an insurance plan that requires a referral, please have the referral forwarded prior to your appointment. Your insurance company will not be responsible for your bill without a referral.

In order to expedite our check-in procedures, please review and complete these forms prior to your visit and email to location you will be visiting Frontdeskdallas @eyesurgeonsdallas.com or frontdeskplano@eyesurgeonsdallas.com.
If you have filled these out in the past, we request that you update these forms so our records are current and correct for each visit.
Authorization for Release of Medical
Please note we ask that you download and complete forms for our patient’s safety and convenience. Complete all registration questions, medical history, and financial policy forms from the comfort of your home by downloading forms. It’s easy just follow and answer the required questions.
Complete all forms and scan/submit via email to frontdeskdallas@eyesurgeonsdallas.com or frontdeskplano@eyesurgeonsdallas.com depending in the location your appointment is in, easy as that. This is HIPAA compliant and safe and will assist in decreasing your wait time and have a safer check-in and updated medical history.
Please allow up to 2 hours for your visit for a complete exam.
Please bring your insurance card, driver’s license (photo ID) and a form of payment.
